Readiness Quiz

This questionnaire not only helps MCF to know more about you, but also helps you to know more about you too! The process of answering the questions prompts you to reflect on your current health status, your current eating and exercise behaviors, where you would like to be with respect to those areas, and your vision for a healthy life overall.

Change is key to getting from where you are to where you want to be. How ready you are to change will greatly impact your ability to successfully reach your goals and realize your healthy life vision.

The Transtheoretical Model of Behavior Change (TTM) was developed by Professor James O. Prochaska, a professor of psychology, and colleagues at the University of Rhode Island in 1977. This model assesses an individual’s readiness to change to new healthier behaviors.

The Stages of Change are:

  • Pre-contemplation – Not Ready to Change Stage
  • Contemplation – Thinking About Changing Stage
  • Preparation – Getting Ready to Do The Change Stage
  • Action – Doing the Change Stage
  • Maintenance  – Have Been Doing the Change for 6 months Stage
  • Relapse – Oops! Fell off the Fitness Wagon;  The “SH$T Happens” Stage

I was doing great, exercising and eating healthy, but then work got crazy, and I fell off the fitness wagon into a fat pothole.
Exercise has become a stress coping mechanism for me. When I’m ready to “pop off” I reach for a dumbbell instead of a pop tart.
I'd like to be healthy but keep falling off the fitness wagon, head first into a 12 piece from KFC. I’m a little iffy on this healthy thing…It could be a bust (again!) but I’m thinking about it.
I have incorporated more fruits and vegetables in my diet in the last 3 months. I’m like the School House Rock poster child!
I was working out, eating healthy, lost weight, but then got bored and gained it all back. How do I end this cycle of yo-yo cray-cray?!
I have started looking into wellness programs (at work, online, gyms) to help me with my diet at exercise. I’m going to do this healthy thing!
I have been exercising and eating healthier for the last 6 months. Feeling and looking good. I have to say, I think I’m kinda dope.
My friends and family are concerned about my weight and asked me to see someone to help with weight loss. They are on me like a tick on a dog’s ass so maybe it may be time to do something about it.
I think healthy eating and exercise are important but I struggle with organizing my life to do these things and would like to work on it. I need help in getting my SH$T together to live healthy.
I was exercising and eating healthy for a big event. I looked fly (if I do say so myself) but then lost motivation and re-gained the weight, which only makes me want to eat more chicken wings!
I have started exercising and eating healthier for the last 3 months. This healthy thing is new but I’m doing the do!
I know I should exercise but I just can’t find the time in my busy schedule. I’m not your super woman(man). How do other people do it?
I’m checking out MCF because I am tired of not feeling well and am ready to change my eating habits. I want to lose some fat so I can let all of my fabulousness come out to play!
I have been exercising three times or more a week for the last 3 months. This healthy thing is new but I think I kinda dig it.
Healthy eating and exercise have been a part of my daily routine for the last 6 months but I’m concerned about maintaining these behaviors. Sometimes the chicken wings be calling me, man, they be calling me!!!
